On the dates hereinafter mentioned, for value received, I, we, or either of us, promise to pay to the order of _________________, the sum of __________________________________ NO/100 DOLLARS ($___________), in lawful money of the United States of America, installments as follows, to wit:

$_________________ due and payable on the _____ day of each and every month thereafter until the entire principal, together with interest thereon, shall have been paid in full. 

with interest from the date hereof at the rate of ____% per annum until paid, interest payable monthly on the installment paying dates, said installment payments to be applied first to accrued interest and thereafter to principal.
 
 

The undersigned shall have the right of prepayment at any time, and all prepayments shall first apply to the interest accrued to the date of prepayment. Any prepayment shall not effect the regularly scheduled payments due under this Note.
 
 

And in case default be made in payment of any installment herein, the entire sum of this note shall become due and payable immediately at the option of the holder of this note. And in case suit or action be commenced to collect this note, or any part thereof, either of principal or interest, I, we, or either of us, promise to pay, in addition to the costs and disbursements allowed by law, such sum as the Court may adjudge reasonable as attorney's fees in said suit or action.
 
 

The makers and endorsers and guarantors of this note severally waive presentment for payment, notice of nonpayment, protest and notice of protest.
